*Annually on June 1st, each renewing member pays a membership fee of $48 ($4 per month). A new music person or group may join at any time paying a set-up fee of $12 plus the pro-rated amount left on the annual fee. (If you wish to be featured in more than one category, please add $10 per additional category.) Upon receipt of this enrollment form and fees, you will be contacted and arrangements will be made to gather the information needed to write your personal listing(s).
